If you have never done it, you will have to redo your region bonuses including Lilith. In extreme mode, you will have escape scrolls, which are hard to find and impossible to craft. You will have to find it in the open world or in the dungeon, be careful it is a white object that is very inconspicuous, keep your eyes peeled! Nightmare Dungeon Affixes
Certain affixes should be avoided or at least spotted before entering the nightmare dungeon:
- explosion of minions when killing them (to be avoided when in close combat)

- killing enemies increases the damage of others
- damage taken increased while vulnerable by 35%

Also remember to store your legendary, sacred and then ancestral level-up equipment so that your new character can benefit from it. Don't pick up your legendaries!
